Agreement to hold harmless, waiver and assumption of risk. (Please print, initial, sign and bring with you)
I verify that my dog is healthy and has received all appropriate vaccinations.
I understand that attending dog training classes is not without risk to myself, my family members or guests who may attend or to my dog because we may be exposed to some dogs who are difficult to control and may be the cause of injury even when handled with care. I agree that I am responsible for myself and my dog and anybody and any things that I bring with me.
I hereby waive and release TymeLes Companions, Leslie Wemhoff, this Training Organization, agents, employees and assistants, and owners and operators of this training location from any and all liability of any nature, for injury or damage which I or my dog suffer, including specifically but without limitation to any injury or damage resulting from the action of any training session, or any other function, or while on the training grounds or surrounding areas.
I hereby agree to indemnify and hold harmless TymeLes Companions, Leslie Wemhoff, this Training Organization, agents, employees and assistants, and owners and operators of this training location from any and all claims by myself or any members of my family or any other person accompanying me to any training session or function or while on the grounds or surrounding areas as a result of any action by any dog, including my own.
